Start at Thai Ministry of Foreign Affairs: official e-Visa portal and identify the embassy or consulate handling applications from your location. Explain that the purpose is medical assessment or treatment and give the expected duration. Ask which category fits your circumstances, including whether a companion needs a separate application.
As a jurisdiction-specific example, the Royal Thai Embassy, Washington DC: Non-O medical treatment visa lists Non-Immigrant O for medical treatment lasting longer than 60 days. Its requirements apply to applicants under that embassy’s rules; do not assume that its document list, fees or processing advice applies worldwide.
Prepare the documents your embassy requests
The Washington DC medical visa checklist includes a passport biodata page, recent photograph, evidence of current location, financial evidence and a signed hospital or medical-centre appointment letter. Separate visa validity, permitted stay and medical readiness
A visa’s validity concerns when it can be used under its conditions. Your permitted stay is the period allowed by immigration for your visit. Neither establishes when you are medically fit to fly. Check all relevant dates on the issued documents and your arrival permission; do not calculate your departure deadline from an appointment letter.

Does a medical visa approve a stem cell procedure?
No. Entry permission does not establish a treatment’s evidence, authorisation or suitability. Those need separate clinical and regulatory checks.
Can the clinic guarantee my visa?
